About The Role

Your Responsibilities

• Conduct one-on-one patient assessments in their homes and occasional virtual assessments
• Perform initial and ongoing assessments to identify physical, psychosocial, and environmental needs of home health patients
• Evaluate patients’ conditions, chart observations, and perform tasks such as vital sign checks and medication reviews
• Train new and existing caregivers to maintain high standards of care
• Assist with business operations to facilitate growth and participate in marketing events
• Share remote on-call responsibilities with the team (not 24⁄7)
  
  

Your Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor's School of Nursing, or vocational nursing program
• Current state license as an RN
• Proof of current CPR certification
• A compassionate and friendly demeanor
• Excellent communication skills
• Current driver's license and reliable transportation
• Ability to work a flexible schedule and travel locally
