Job Summary and Essential Job Responsibilities : Under general supervision, provides intensive case management services within a team setting to a group of individuals with severe and persistent mental illness. The case manager will provide outreach, advocacy and support services to clients in Ravalli and surrounding counties to include assisting clients to identify strengths, needs & goals in residential, health, vocational, educational, social and economic domains. Coordinate necessary specialized services including psychiatric, psychological, medical, educational and housing based on the needs and desires of the client. Facilitate client access and utilization of natural community resources. Acts as an advocate on behalf of the client and insure the protection of rights through direct intervention, education and facilitation of advocacy groups in the community.
Qualifications : Considerable knowledge of the principals and practices of social work, human development and behavior; of contemporary social & economic conditions; working knowledge of laws, rules & regulations related to individuals with mental illness; and of the needs and problems related to individuals with severe and persistent mental illness. Must have valid driver’s license, auto insurance with 100/300 bodily injury coverage, and reliable transportation.

Education & Experience : Bachelor’s degree in Social Work or Human Services Related field preferred, minimum one year of full-time experience serving people with mental illnesses.'
