Bereavement Services Director - Home Care designs, plans and directs the home care bereavement program. Trains and coordinates a staff of bereavement counselors. Being a Bereavement Services Director - Home Care assigns counselors to families as necessary, and ensures services fulfill the needs of the family. Requires a bachelor's degree. Additionally, Bereavement Services Director - Home Care typically reports to top management. The Bereavement Services Director - Home Care manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. Extensive knowledge of department processes.     Responsibilities- RN Field Supervisor:

    • Medication administration and compliance
    • Wound care as ordered by MD
    • Nursing procedures per physician orders and agency policy and procedure.
    • Admissions, Recertification, Resumptions of Care, and Discharge Assessments of patients in need of home health skill services. Establish plan of care that are consistent with medical records from hospital or MD notes.
    • Prepare clinical and progress notes for individual patients to include physical and mental conditions of clients as well as signs and symptoms indicative of changes in condition
    • Have the ability to follow the Physician’s orders in evaluating and caring for the patient and advise Physician of any changes in the patient's condition or needs
    • A graduate of an accredited school of nursing and a current state nursing license
    • One (1) year nursing experience recommended, experience in home health a plus
    • Ability to provide and execute exceptional patient care
    • Supervise Field staff to ensure care is accordingly as per the established plan of care.
    • Coordinate patient care with Clinical Mgr., field staff, and MD offices to ensure client's needs are met.
