Patient Care Navigator-Surgical

  • Patient Care Navigator- Surgical (Non-Clinical) is adept at working with patients to help them find solutions to their healthcare specific to surgical scheduling needs and help to navigate the healthcare system when a procedure is imminent to ensure that they can keep appointments, understand the process, and have open communication with their providers.

    Duties and Responsibilities (including but not limited to):

    • Identifying, facilitating and securing access to needed healthcare, social services benefits and community resources
    • Using decision support tools and supervisory support to identify appropriate interventions and health care and social service needs
    • Communicating with clients, their families and caregivers to support care plan goals and integrate care delivery
    • Working with hospital surgical staff to ensure that the information they require to book a surgery appropriately and to increase the likelihood that a procedure will be performed as scheduled.
    • Facilitating preoperative appointments or any other appointment necessary to prepare and clear a patient for a surgical procedure
    • Track outpatient appointments to ensure patient compliance and rescheduling as necessary
    • Regularly coordinating and communicating with Care Team members for purposes of discussion surrounding patient co-morbidities which need to be managed due to a surgical procedure
    • Working in collaboration with Care Team Members and hospital staff to ensure that the necessary and applicable forms, consents, testing, appointments, and medication adherence is communicated with all parties and all applicable documentation is shared with all applicable parties
    • Collaborate with the patient and patient representatives on specific patient care needs
    • Communicates and collaborates with hospital staff to ensure that metrics surrounding completed surgical procedures are improved and any opportunities for additional support that can be offered is discussed
    • Meet with the team regularly to discuss improvement of outcomes and adjusting to the population’s need
    • Maintains logs and other data bases regarding care coordination activities

     Qualifications:

    Education and Experience:

    High School degree or equivalent

    1-2 years’ experience in a healthcare setting  

    • Medically-based clerical experience (previous surgical scheduling or navigation experience preferred)
    • EMR/EHR (Cerner/PowerChart/Medent/HealthENet/HealthELink)
    • Microsoft Office (Excel/Outlook/Word)
    • Ability for Collaboration with Team
    • Some Medical Terminology Preferred

      Specialized Knowledge & Skills Needed for Performance of Job:

    • Excellent customer service skills-ability to understand/exceed customer expectations while demonstrating the highest standards of care, respect, & confidentiality
    • Must be able to work independently/alone and demonstrate self-motivation
    • Comfortable with regular patient interactions
    • Knowledgeable on how to navigate all aspects of medical care and managed care system
    • Excellent communication skills both verbal and written
    • Ability to handle multiple task and priorities
    • Exceptional organizational skills

     *This is a hybrid-remote position and candidates must reside in WNY to be considered.

    We offer an outstanding benefits package including health, dental, 401K, vacation, and PTO, as well as a great working environment 

    Pay range $18.00 to $23.00

    The referenced pay range represents the minimum and maximum compensation for this job.  Individual annual salaries/hourly rates will be set within job’s compensation range, and will be determined by considering factors including, but not limited to market data, education, experience, qualifications, and expertise of the individual, and internal equity considerations.
