Job Description:
The Care Navigator at Next Level Medical is a critical member of our healthcare team, responsible for guiding patients through their healthcare journey, providing education, coordinating care, and ensuring the best possible outcomes. Reporting to the Care Navigator Manager, this position combines clinical expertise and compassionate communication to support our core values of Service-Excellence, Teamwork, Integrity, Innovation, and Compassion.
Responsibilities:
- Act as the primary point of contact for patients, ensuring timely and appropriate communication, education, and support throughout their healthcare journey
- Collaborate with physicians, nurses, and other healthcare providers to develop and implement individualized care plans for patients
- Coordinate appointments, tests, and procedures to ensure seamless care delivery and avoid unnecessary delays or duplication
- Provide patient education on medications, treatment plans, and lifestyle modifications to promote optimal health and well-being
- Monitor patient progress, adjusting care plans as needed and communicating any changes to the appropriate healthcare providers
- Maintain accurate and up-to-date patient records, documenting all care coordination activities and interventions
- Participate in ongoing professional development and training to stay current with best practices in care navigation and patient support
Requirements:
- Current, unrestricted licensure as a Registered Nurse (RN)
- 2 years of clinical experience, preferably in care coordination, case management, or a related role
- Excellent communication, interpersonal, and organizational skills
- Strong ability to work collaboratively with patients, families, and healthcare providers
- Proficient in electronic health records (EHR) systems and Microsoft Office Suite
- Commitment to providing compassionate, patient-centered care in alignment with our core values
