Case Manager - Alcohol or Drug jobs in Idaho Falls, ID

Description

1. Assesses clients for need of in-home care and requests care.
2. Coordinates with Nurse Coordinator and physician for all necessary visits to obtain/ maintain in-home care.
3. Coordinates nursing care for the client once approved by the Department of Labor for nursing, home health aide, and applicable therapies.
4. Maintains confidentiality of all office and client records per HIPPA guidelines and Privacy Act PHI.
5. Maintains effective communication and collaboration with entire care team on behalf of the clients.
6. Performs other duties as assigned.

Qualifications

1. Current registered nursing license in good standing.
2. At least one-year experience in an acute/chronic medical field or related with good assessment skills.
3. Management skills are a plus.
4. Good organizational and time management skills to manage multiple clients at one time.
5. Good working knowledge of computer and internet-based applications along with other office machines as necessary to complete job.
6. Competent working knowledge of client-based equipment.
7. Must pass a background check and sanction screening.
8. Professional appearance necessary.
