Case Manager - Alcohol or Drug jobs in Pennsylvania

Malibu Wellness Ranch in Pike County is a therapeutic equine inpatient rehabilitation program in Milford, PA for those in recovery. We are currently seeking for immediate hire for a Case Manager. Shifts: 8am-4pm.

We utilize the animals on the ranch and horses to help those through the recovery process, while providing them with treatment programs throughout their wellness stay.

Job responsibilities include

• Screen and assess
• Collaborate with other team members to ensure individual needs are met
• Work with local probation, state parole and county correctional facility to ensure individuals' needs are met upon re-entry into the community
• Collaborating closely with drug and alcohol counselors to facilitate discharge planning.
• Facilitate transitional phase of the program for clients, preparing for discharge.
• May arrange medical appointments.
• Perform administrative duties related to a client's care, such as filling out forms for social services benefits and arranging other services as needed throughout the duration of the client's residence in the facility.
• etc

One year of experience working with substance abuse clients, preferred. CPR/First Aide certification a plus.
