We are seeking a dedicated and experienced Registered Nurse Case Manager to join our healthcare team. This position requires expertise in pediatrics, medical records, managed care, and utilization management.
Duties:
- Conduct comprehensive assessments of patients' medical conditions and develop individualized care plans
- Collaborate with healthcare providers to ensure effective communication and coordination of patient care
- Monitor patients' progress and make necessary adjustments to the care plan as needed
- Provide education and support to patients and their families regarding their medical conditions and treatment options
- Coordinate discharge planning and ensure smooth transitions between healthcare settings

Experience:
- Valid Registered Nurse (RN) license

- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
