Case Manager coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). Case Manager's years of experience requirement may be unspecified.     POSITION SUMMARY: Provides support and advocacy to clients and their families or caregivers within residential program to educate, provide opportunities for healing and recovery as well as future success.

    ESSENTIAL POSITION DUTIES AND RESPONSIBILITIES:

    • Facilitates Human Trafficking Curriculum to youth at residential program.
    • Provides population and age-specific case management services to clients, including assessment, linking/coordinating, monitoring, reassessment, follow-up, service plan development, resource development, transportation, and advocacy.
    • Participates with staff in the development review and the delivery of treatment plans. Leads and coordinates the treatment team process.
    • Provides brief clinical management of individuals and/or families in crisis and, when necessary, conducts emergency crisis intervention.
    • Provides support to clients and families in identifying and accessing services as decided by the treatment plan.
    • Maintain required case management documentation.
    • Coordinates services on behalf of clients and/or families with supportive agencies and/or caretakers.
    • Participates in in-service training, clinical supervision, red-flag meetings, and other meetings.
    • Participates with staff and conducts client group meetings related to substance abuse and mental health recovery.
    • Provides transition and aftercare planning for clients.

    11. Completing Discharge summaries.

    12. Learning and training the behavior motivation plan to new hires.

    13. Uploading new client’s information and uploading case management documentation such as email invitations to treatment into medical records system.

    14. Track and complete the 6-month aftercare follow up for discharged clients.

    KNOWLEDGE AND EDUCATION: Bachelor’s Degree in a related mental health or child welfare services field.

    WORK EXPERIENCE: One to two years of progressively more responsible experience in a related mental health, behavioral health, and substance abuse services field with at-risk children/youth is preferred.

    PHYSICAL DEMANDS:

    • Occasional requirements: Lifting, pushing, pulling, and carrying up to 25 pounds. Climbing, kneeling, stooping, and bending.
    • Frequent requirements: Sitting and walking for long periods of time. Short/medium length car trips, getting in and out of the vehicle, going up and down the stairs.
    • Therapeutic Crisis Intervention Training

    MENTAL DEMANDS:

    • Frequent requirements: Attention to detail/concentration on figures, paperwork, data, etc. Pressures related to meeting deadlines, fulfilling scheduling requirements, etc. Simultaneous handling of multiple projects. Distracting influences such as people, telephone calls, noises, and so forth. Intermittent or cyclical work pressures.

    Job Type: Full-time

    Pay: $38,000.00 - $41,000.00 per year
