Case Manager - Mental Health Services jobs in Chillicothe, OH

We are seeking a dedicated and detail-oriented Case Manager/Discharge Planner to provide comprehensive case management, screening, and assessment, and works with a multi-disciplinary team to coordinate services for clients in the jail system. Provides direct assistance, psycho-education, relapse prevention skills, and advocacy to assist clients in obtaining appropriate resources to improve client outcomes and quality of life after being released from jail.

Minimum Requirements: Bachelor’s Degree preferred or Associate’s degree in Human Services or related field with three years related experience and/or training; or equivalent combination of education and experience preferred. LSW or LPC required. Three years’ experience in the mental health field required; Experience working with individuals experiencing SUD and COD concerns preferred. Valid Ohio Driver’s license, use of personal car, and annual training standards as required by the Center.
