We offer a range of services including Case Management, ARMHS, CTSS, and individual, and family therapy. The Mental Health Case Manager is responsible for providing comprehensive case management for Adult with persistent mental illness. This individual will help them gain access to medical, social, educational, vocational, and other necessary services connected to the person’s mental health needs.
Duties and Responsibilities:
· Meet Monthly with client and provide on-going support and case management expertise to adult with severe and persistent mental illness
· Perform assessments on each new client including: Individual Community Support Plan (ICSP), Functional Assessments, LOCUS, etc. addressing client’s needs, deficits, strengths, and goals.
· Continually monitor and evaluate each client’s progression through their case management plan, and develop corrective action revisions to the plan as needed
· Conducts crisis intervention as needed.
· Communicate with case management team members, community professionals, and other service providers to ensure that services for clients are coordinated.
· Completes periodic ICSP, Functional Assessments, LOCUS at least once every six months or as significant changes occur
· Monitors client’s progress and meet with them face to face at least once monthly
· Ensures the medical record is up to date, accurate, and meets all documentation requirements.
· Advocates for clients as appropriate
· Participate in clinical supervision and takes guidance and direction from the supervisor.
