Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work. Natchaug Hospital is Eastern Connecticut's leading provider for children, adolescents and adults with mental illness and substance abuse addictions. We offer a variety of programs including inpatient and outpatient behavioral health, residential treatment for court-involved teenage girls, geriatric services, clinical day treatment schools and chemical dependency services. Through our multi-site network of care, we constantly strive to meet its mission in helping people find their way while educating and empowering individuals to participate in their own care and recovery.
Job Summary
The Case Manager is responsible for providing a range of services for clients with either Psychiatric and/or Substance Abuse issues. Also provides advocacy, outreach, and service linkage for clients and initiates access to entitlement services.
Qualifications
- Experience working with a behavioral health and/or chemical dependency population
- Bachelor's degree in Human Services or related field, Social Work preferred
- Valid driver's license may be required based on program
- Knowledge of community resources preferred.
- Computer proficiency and keyboarding skills required
