Our Outpatient team has a fantastic opportunity for a Clinical Case Manager.
As a Clinical Case Manager, you will work as part of a team to provide outpatient mental health services and community support. You will be responsible for writing treatment plans and reviews in a collaborative format and work with clients to identify goals and methods for success. Our Case Managers work with a case load of adults with severe and persistent mental illness providing treatment based on client need that includes identifying and using community resources, liaison with other member treatment providers, referral to other appropriate community programs, and life skills training.
