Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work.     What You’ll Do as a Case Manager:
    We are currently looking for Mental Health Case Managers.

    Care facilitators help youth live healthier and more fulfilling lives through identifying and supporting their needs and goals. Care facilitators specialize in:
    • Managing the care provided to youth in the residential setting
    • Helping youth gain access to the services they need to meet their goals
    • External collaboration with community partners
    Primary duties as a Case Manager:
    • Assessment of the eligible individual to determine service needs
    • Development of a specific care/treatment plan
    • Referral and related activities to advocate and assist the individual obtain needed services
    • Monitoring and follow up of the plan
    • Evaluation (of impact of treatment plan on recovery)
    • Interact frequently with the person’s treatment team including residential therapists, physicians, other treatment providers involved, community agencies, referral sources and other services.
    • Linkage and networking with DCS, court, schools, health care providers, employers and other community agencies
    • Provide integration and on-going assistance to youth and families to help sustain within the community/least restrictive environment
    • Accountable for the flow of information within a multidisciplinary team setting
    • Special Populations/Skills/Employment duties, as assigned
    • Hours may include availability in the evenings and/or weekends

    Minimum Hiring Requirements for a Case Manager:
    • BA/BS in social service or a human services-related field required.
    • Two years’ experience in a social service setting preferred
    • Must have a valid driver’s license and proof of insurance
