Case Manager - Mental Health Services coordinates the overall plan of care for patients with mild to moderate emotional and behavioral problems. Oversees patient evaluation and treatment planning, and monitors patient progress, adjusting treatment plans as necessary. Being a Case Manager - Mental Health Services acts as a liaison between patient/family and service delivery personnel to ensure patients are being effectively treated. Typically requires a master's degree in social work or equivalent. Additionally, Case Manager - Mental Health Services typically requires a State License to Practice Clinical Social Work. Typically reports to a head of a unit/department. The Case Manager - Mental Health Services occasionally directed in several aspects of the work. Mental Health Professional I- Elder/Adult Services

    This position is located at the Department of Mental Health, Santee-Wateree Community Mental Health Center (SWCMHC), Sumter Clinic, 801 N. Pike West, Sumter, SC 29153.  

    Under the limited supervision of the Sumter County Clinic Director, formulates and executes specialized mental health services to a specialized Elder Services Population.  Performs advance counseling in the complex areas of mental health, provides case management services to include assessment, crisis intervention, individual, group and family therapy, and attend staff meetings.  Represent the Center and program in the absence of the Program Coordinator.    
    Responsibilities for this Position  include but are not limited to:
    1. As mental health clinician provides direct care services such as conducting individual, group and family therapy sessions and other services as needed such as crisis mgt., assessments, Triage, participates in team meetings and formulation of Plan of Care, Safety Plans and DLA-20.  Promotes and educates clients on coping skills, symptom management and behavioral management tech that will aid in improved functioning and community. Monitors for symptoms and compliance. 
    2. Assures that the specialized Elder/Adult Services population documentation and medical records for the clients conforms to DMH, local state and federal regulations as well as corporate compliance and HIPPA. 
    3. Work independently in the absence of Coordinator, forms and maintains a good professional working relationship with colleagues and other services provider agencies in the community to enhance consumers linkage to needed services and opportunities.
    4. Travel throughout the Santee- Wateree CMHC catchment area as directed. Performs other related duties as required.
    State Minimum Requirements:
     A Master’s degree in guidance, counseling, the social sciences, or a related field.

    Licensed or licensed eligible.
