Job Summary:
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, conducting assessments, and collaborating with healthcare providers to ensure high-quality care delivery.
This position is an on-call/after hours home health RN Case Management position. You will be compensated $50/weekday 5pm-8:30am $100/weekend 24 hours. If you receive a call, you will be compensated a visit rate on top of the on-call rate. The area of coverage will be the following counties: Berrien, Van Buren, Cass, St. Joseph, Kalamazoo, Calhoun, and Branch.
Responsibilities:
- Manage patient cases from admission to discharge, ensuring appropriate care plans are implemented
- Conduct comprehensive patient assessments, including vital signs monitoring and behavioral health evaluations
- Collaborate with physicians and healthcare professionals to develop and implement individualized care plans
- Monitor patient progress and update care plans as needed
- Provide patient education on treatment plans, medications, and self-care techniques
- Maintain accurate and up-to-date medical records
Skills:
- Experience in managed care and patient care coordination
- Proficient in patient assessment techniques and vital signs monitoring
- Knowledge of geriatric care and behavioral health assessments
- Familiarity with medical terminology and physiology
- Strong organizational skills and attention to detail
