Case Manager (Non-RN) coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Case Manager (Non-RN) typically requires a bachelor's degree. Typically reports to a head of a department/unit. To be a Case Manager (Non-RN) typically requires 2 to 4 years of related experience. This is a temporary position, scheduled to go through the middle of September.
Specific Duties:
1. Hospital Transition calls for Medicare Advantage patients
2. Care Coordination for complex, high-risk patients
3. Motivational interviewing with patients and families
4. Palliative Care conversations when appropriate
5. Participation in Interdisciplinary Team meetings
Must have qualifications/experience:
1. Ambulatory Care
2. Outpatient Care Coordination
3. Difficult conversations
4. Care Management
5. Hosptial Transition work
Ability to engage with complex patients during complex care transitions. Working top of scope - coordinate next steps and plan of care with autonomy.
There are no certifications required for the role (but CCM or Palliative Care Certification would be amazing!)
Start & end times of shift: 7:30-4 pm, M-F
We have occasional team meetings in a different location of Everett
