Hospice by Loving Care is the Hospice division of Loving Care In-Home Health Services. We are a home health, hospice and private duty services provider since 1997. Our health care services are available to people in 16 counties throughout central Oklahoma.
This position will do visits out of the Noble Hospice branch location. The Hospice RN Case Manager position is responsible for the provision and documentation of hospice services to patients in accordance with an established interdisciplinary plan of care. This position will complete visits in Noble, Norman, Moore and OKC metro surrounding areas. This position requires the following:
