Case Manager, Sr. coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager, Sr. evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager, Sr. identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). **Job Summary:**
The Case Manager will provide case management, coordinate services, and counsel individuals. They will assist in accessing entitlements, benefits, and community resources to reduce dependence on emergency assistance programs.
**Responsibilities:**
- Counsel on a short-term basis and provide crisis intervention services.
- Assess client suitability and assist in completing forms for entitlements and supplemental insurance.
- Assist in accessing prescription cost assistance programs.
- Advocate for clients when interacting with community agencies.
- Maintain accurate client records and prepare necessary documentation.
- Coordinate services such as transportation assistance and prepare client correspondence.
- Identify the need for client education and support services.
- Consult with Providers and make referrals as needed.
- Evaluate and address the needs of special population clients.
- Provide necessary counseling and testing services and refer individuals as appropriate.
- Follow up on missed appointments and assist with client flow.
- Maintain a clean and safe work area.
- Attend clinical and team meetings.
- Perform any other relevant duties as assigned.
**Qualifications:**
- Bachelors Degree in Social Work or related field.
- Two years of experience as a Case Manager with strong interpersonal and communication skills.
- Computer literacy and proficiency in MS Office.
**Preferred Qualifications:**
- Five years of experience as a Case Manager.
- Experience working with diverse populations.
**Physical Demands:**
This position requires periods of sitting, standing, walking, bending, and stooping. Weekend and evening work may be required.
**Salary Status:**
Hourly; non-exempt.
Pay Range: $20 - $24/hr.
