Case Manager, Sr. coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager, Sr. evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager, Sr. identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). The Case Manager, Sr. contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature.     Position Summary:

    The Case Manager (Data) serves as a member of the Prevention team of WestCare Tennessee and works to ensure the delivery of quality services to their coworkers and participants within the program. This role provides case management to eligible participants in suicide prevention, awareness, and community resource linkage.

    Essential Job Functions:

    • Embraces and embodies the mission, vision, guiding principles, and goals of WestCare Tennessee.
    • Uphold strict confidentiality of participants information.
    • Works in collaboration with the Counselor to provide services to participants.
    • Review Individual Service Plan (ISP) for participants.
    • Review the Wellness Recovery Action Plan (WRAP) with participants.
    • Participates in the development of individualized resident care plans.
    • Develop and review recovery treatment plan and assessments.
    • Score assessment instruments with guidance from the Project Director.
    • Complete a service plan for continued peer support and family intervention activities.
    • Participates in clinical staffing decisions regarding the participants current strengths and resources and their need for additional resources.
    • First (walk-in) or second initial contact (after referral sent by Peer Support Specialist) with eligible participants.
    • Schedule appointments with eligible participants to complete program eligibility process and review required documentation.
    • Facilitate one-hour weekly case management meeting with eligible participants by either virtual platform or telephone.
    • Assist eligible participants with accessing community resources, employment (job readiness, resume and job search), housing assistance, and other needed services.
    • Maintain strong professional relationships with organizations and other community agencies to receive referrals and utilize office space for meetings and group activities.
    • Maintain strong professional relationship boundaries with participants served and organizations/ community agencies who refer individuals to WestCare Tennessee.
    • Assist in implementing a public health approach that blends community-based prevention with evidence-based clinical strategies through community efforts.
    • Promote and provide health and wellness of eligible participants.
    • Establish, develop and maintain positive working relationships with community partners that include the school system, law enforcement, local, federal, state, grantors, community stakeholders and other businesses and agencies.
    • Educate and refer participants to community agencies and businesses that appropriately address the participant’s needs for food, shelter, clothing, employment, transportation, etc.
    • Assist participants with establishing or re-establishing support systems within their community.
    • Advocate on behalf of the participants to improve access to needed services.
    • Collect baseline, discharge, and follow-up data on participants.
    • Coordinates discharge and/or transfers to ensure appropriateness and continuity of care.
    • Maintain tracking and follow-up logs for participants.
    • Maintain all case files and input data in tracking system.
    • Engage and build rapport with participants to assist with tracking and follow-up.
    • Assist with meeting participants’ psychosocial needs.
    • Completes mandatory documentation within timelines established by applicable regulations.
    • Attend in-person and virtual program conferences and meetings, community meetings, and events as deemed necessary.
    • Attend necessary monthly integrated staff team meetings in coordinating the care and services for each enrolled veteran and their families.
    • Serve as a representative of WestCare Tennessee at community events, meetings, workshops and activities.
    • Submit completed trainings and professional development trainings to the Project Director.
    • Submit weekly reports on activities to Project Director on case management outcomes.
    • Work closely with Project Director, Counselor and WestCare Foundation Evaluation team regarding the collection of data and reporting of program outcomes.

    Essential Qualifications:

    Certifications/Licenses:

    • Valid driver’s license is required.

    Education:

    • Graduation from an accredited two-year Community College or a four-year College/University with a degree in Communication, Sociology, Criminal Justice, Social Work, Psychology, or related human services field to the position is preferred.
    • Current and/or previous experience in a behavioral health service-related field of at least 3-5 years will be considered in lieu of a degree.

    Experience and Competencies:

    • Basic knowledge of veteran services, behavioral health, social services, judicial system, law enforcement or related industry.
    • Experience with case management and administering assessments preferred.
    • Excellent communication skills including public speaking and group facilitation/training.
    • Some experience with performing outreach and marketing.
    • Ability to exercise good judgment and discretion.
    • Ability to work well in a team environment.
    • Ability to adhere to the highest standard of ethical conduct and confidentiality.
    • Professional appearance and demeanor.
    • Must be culturally/linguistically sensitive to populations served.
    • Research and evaluate data.
    • Experience with using data tracking systems.
    • Ability to work in different program systems in providing day to day operations.
    • Ability to stay highly organized and maintain time management.
    • Ability to utilize a computer and software that includes Microsoft 365.

    Working Conditions:

    • Work is primarily performed in an office or indoor setting.
    • Some outdoor activities and travel will be required.
    • Some travel will be required to trainings and conferences.
    • Regular and prompt attendance is required throughout employment.
