Case Worker - Home Care manages cases regarding utilization review, discharge planning, and patient services coordination. Collaborates with insurers, managed care organizations, referral providers, patients, and families to assist developing case management guidelines. Being a Case Worker - Home Care may require a bachelor's degree. Typically reports to a manager. The Case Worker - Home Care gains exposure to some of the complex tasks within the job function. This position is able to be 1099 contractual, our hourly. We are open to candidates who are interested in full time or salaried.
An ideal candidate has a bachelors degree, ability to have a flexible schedule to include some daytime hours, some evening hours, and occasional weekends depending on caseload needs. An ideal candidate would be comfortable providing casework services within the community and within the clients home throughout Koscuisko County, Indiana.
Clients we provide services for can be children, teenagers, and adults. We have clients who are involved in court ordered services through Department of Child Services, Probation, and/or Drug Court. Services provided can include homemaking, parenting education training, supervised visitations, and casework services for our clients.
Responsibilities:
- Provide services in the clients home, community, or office setting. Clients may consists of children, adults, or teenagers.
- Counsel patients issues and provide support for their mental well-being
- Perform patient assessments to develop individualized treatment plans
- Adhere to HIPAA regulations and maintain confidentiality of patient information
- Collaborate with other healthcare professionals to ensure comprehensive care for patients
- Particpate in ongoing supervision and training with assigned supervisor. Receive mentoring and supervision from highly trained leaders.
- For DCS and Probation clients, attend regular team meetings about client progress and attend court hearings
Qualifications:
- Bachelors degree in Human Services
- Familiarity with support services
- Strong communication and interpersonal skills
- Ability to maintain confidentiality and adhere to ethical guidelines
- Ability to drive and have a valid drivers license
- The ability to pass a criminal history and child protective services background check.
