Job Overview:
We are seeking a compassionate and dedicated Home Care Aide to provide personalized care to vulnerable adults within their homes. The ideal candidate will have experience in dementia care, light housekeeping, and direct support.

Responsibilities:
- Provide high-quality care in participant homes
- Assist with activities of daily living including meal preparation, cleaning, and personal hygiene.
- Provide medication reminders as prescribed by the physician
- Monitor and document participant condition and progress
- Collaborate with healthcare professionals to ensure optimal participant care when needed.
- Assist with mobility, including lifting and transferring patients when necessary

Skills:
- Experience in dementia care and home care
- Proficient in meal preparation and cleaning tasks
- Strong caregiving skills with a focus on participant well-being
- Knowledge of medication administration systems
- Previous experience in Assisted living or direct support roles
