Overview
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, advocating for patients, and collaborating with healthcare professionals to ensure the best possible outcomes.

Duties
- Develop and implement care plans for patients
- Monitor and evaluate patient progress
- Coordinate with healthcare providers to ensure continuity of care
- Educate patients and families on treatment plans
- Advocate for patient needs and rights
- Maintain accurate and up-to-date medical records
- Collaborate with interdisciplinary teams to optimize patient care

Requirements
- Current Registered Nurse (RN) license
- Experience in infectious disease care, critical care, or geriatrics preferred
- Proficient in medical terminology and HIPAA regulations
- Strong knowledge of physiology and ICU practices
- Ability to work effectively in an inpatient setting
- Excellent communication and interpersonal skills
