MDS Coordinator Responsibilities You'll Love:

• Completing accurate MDS assessments per RAI coding guidelines

• Initiating and maintaining care plans and supporting Interdisiplinary team as assigned

• Creating and distributing monthly care plan calendars in a timely fashion

• Monitoring & auditing clinical records, ensuring accuracy & timeliness

• Coordinating optimal quality patient care together with the IDT

• Protecting the confidentiality of Resident & Facility information at all times

• Tracking and optimizing CMS 5 star ratings and quality measures

• Closing out monthly billing timely and managing weekly utilization review meetings

• Other duties and essential functions of the MDS coordinator role as assigned

What We Look For:

• Valid RN license.

• Prior MDS 3.0 experience is required.

• Previous long-term care experience is a must.

• Ability to work 40 hours per week.

• Highly organized, professional, and self-motivated.

• Strong computer skills.

• Excellent communication skills, both verbal and written.

• Friendly, approachable, and a strong team player.
