DESCRIPTION
Youth Navigator for Substance Use Dependency is responsible for outreach implementation and program enrollment. This position will provide culturally responsive outreach strategies specific to youth/young adults in their communities, strategies to create youth-friendly environments, and pathways toward sustainable programs, from outreach to recovery support. The ideal candidate will be fluent in English and Spanish and will have experience working with various population including vulnerable youth.
PROGRAM’S MISSION
Youth Opioid Reduction (YOR3) is committed to enhancing and expanding access to youth-relevant outreach, MAT, and other prevention, intervention, treatment, and recovery services.
YOR 3 is dedicated to strengthening capacity and access prevention, treatment, and recovery services, as well as access points to Medication-Assisted Treatment (MAT), for youth (ages 12-24) and their families. Our mission is to suppor youth who are experiencing many challenges related to substance abuse. The program focuses on outreach and engagement, overdose prevention, and creating comprehensive care for youth/young adults with opioid use disorder (OUD) and/or stimulant use disorder (StUD). This program will work out of Oakhurst and the city of San Joaquin to inform, support, empower and equip youth with the tools, knowledge, and confidence to pursue healthier choices for themselves and their futures.
QUALIFICATIONS
· Candidates should have an Associate Degree or be working towards a degree in social work, human services, or related field. Three years of proven case management work may be substituted for education requirements. Knowledge of Word, Excel, and other computer programs desired.
· Knowledge of alcohol and drug prevention and recovery/treatment programs/or lived experience.
· Knowledge of the effects of drug use and alcohol
· Must have a driver’s license
· Must be fluent in English and Spanish and possess excellent writing skills in English.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Conducts culturally sensitive outreach in the community to enroll clients
Develop and implement educational activities for youth and family members
Conducts initial intakes and assessments
Create treatment recovery plans for program participants
Establish rapport with transition-aged youth from diverse backgrounds and experiences.
Work with youth and/or families to link youth with resources that are appropriate to their individualized needs.
Create individualized plans, provide coaching, and resource navigation services related to educational and vocational services, health and wellness, crisis intervention, leadership development support, etc.
Monitor participant progress and maintain accurate records including data entry into approved database and reports.
Reports to work on time and maintains reliable and regular attendance.
Performs other duties as assigned.
