Chief CAT Scan Technologist oversees computer-assisted tomography and the maintenance of department supplies, equipment, and facilities to ensure the quality of services.   • Community Health Coordinator
    The Community Health Coordinator will report to the Outreach Supervisor. The Community Health Coordinator will be responsible for families across a continuum of services. They will complete intakes, assessments, developmental screens, and provide ongoing case management services to a variety of families accessing services. They will have the ability to have a quick response to families and medical centers to provide services and information. Families are served in a medical setting, community, in the office, and in their homes. This position will be eliminating barriers for families to access services including mental health care and medical care. They will oversee the daily operations at our partnering site.
    ESSENTIAL FUNCTIONS
    • Maintain a caseload as referrals are available to meet productivity requirements.
    • Successfully deliver services to referred families.
    • Conduct creative and persistent outreach to engage families into services (including, but not limited to, face-to-face visits, drop-bys, phone calls/texts).
    • Provide intake and assessment of all families on case load within required timelines.
    • Complete case notes in a detailed, yet succinct, finished format.
    • Maintain data, including demographics, referral sources, and medical visits.
    • Maintain and create data systems including visit summaries, demographics, assessments, etc.
    • Collaborate with partners inside the organization and in the community.
    • Complete scheduled and unscheduled appointments.
    • Complete developmental screens.
    • Establish an emotionally supportive, non-judgmental, trusting relationship with parent(s), using independent judgment and adjusting as needed.
    • Independently establish a case plan with goals, objectives and activities with each client, reviewing and rewriting, as well as setting new goals as circumstances change. This case plan will incorporate the needs identified by the referral source as well as the needs identified by the staff and the client. This case plan will guide services.
    • Assist clients with achieving goals and objectives set forth in case plan within time frame established.
    • Assist families with medical and follow up appointments as needed.
    • Exercise discretion and judgment in identifying plans of action to achieve goals and objectives, including day-to-day direct contact with clients, government entities, other agencies, and courts.
    • Advocate on behalf of the client and link the family to additional community resources and services.
    • Complete all required paperwork in various databases within established format and deadlines.
    • Work within an environment of frequently changing priorities, deadlines and duties.
    • Provide client transportation as necessary to complete goals of the program and client case plan.
    • Assist with crisis assistance as needed, such as hands-on lice treatments, home cleanliness, and same day responses to family needs such as mental health treatment coordination and homelessness.
    • Assist with crisis assistance in local hospitals and medical centers to wrap services around families.
    • Teach and role model parenting techniques to the family using approved methods and materials.
    • Teach home management, financial management, childcare, discipline, nutrition and personal skills as appropriate.
    • Report any case that may involve “imminent harm” or “threatened harm” to a supervisor immediately.
    • Build relationships with partnering staff.
    • Attend court hearings, case conferences, team meetings, department meetings, and other meetings as required
    • Develop, coordinate, and execute informational presentations at conferences, speaking engagements, etc.
    • Research and develop curriculum and resources for child abuse prevention education topics.
    • Collaborate with community leaders to create innovative opportunities for program possibilities and prevention services.
    • Maintain minutes from community meetings and communicate information throughout the organization.
    • Educate and train families, educators, providers, and partnering staff.
    • Assist with cultivating relationships with individuals and organizations to further the mission of the organization and promote SCAN services.
    • Perform outreach and marketing activities as needed to establish community relationships and understanding about services offered.
    • Attend 12 hours of in-service training sessions per year.
    • Meet with supervisor for supervision on a weekly basis.
    • Attend team meetings, department meetings, and other meetings as required.
    • Performs other duties as assigned
    • Approximately 40% travel is expected for this position. Must be able to travel within all counties in the SCAN service area.

    TRAVEL
    • Travel is expected for this position, including daily travel within the service area to complete client visits and occasional travel within the state for required trainings and meetings.
    EXPECTED HOURS OF WORK
    • Some flexibility in hours is allowed, but the employee must be available between the "core" work hours of 8am to 8pm Monday-Friday and 10am to 2pm on Saturdays and must work 40 hours each week to maintain full-time status.
    REQUIRED EDUCATION
    • Bachelor’s Degree in Human Services, Social Work, Education or directly related field.
    • One year of full-time experience in home management, home maintenance, and/or child-care.
    • Experience working with families/parents/children in a human service organization a preference.
    • Life or job experience caring for children.
