Job Description
Overview :
The Case Manager performs routine case management work. The Case Manager develops and maintains contact with clients, client families and other service providers in order to coordinate the medical, clinical, social and related needs of clients during treatment. The Case Manager acts as a member of a multi-disciplinary team of treatment providers. This is a full time 40 hour position.

Experience in social work. Bachelor’s Degree in related field. Education and experience may be substituted for one another.
