Clinical Facilities Planner offers professional services on assigned projects concerning design, construction, development, or installation of hospital buildings and facilities. Oversees construction progress, ensuring that timing, costs and contract requirements are adhered to. Being a Clinical Facilities Planner supervises construction, equipment installation, renovation and redesigning projects. Works with hospital departments to review design changes, equipment specifications/substitutions, purchasing and accounting procedures. Additionally, Clinical Facilities Planner develops and designs feasibility studies for review. May require a bachelor's degree. Typically reports to a manager or head of a unit/department. The Clinical Facilities Planner contributes to moderately complex aspects of a project. Work is generally independent and collaborative in nature.   • Duties

    • Consult with the Primary Care Manager on behavioral health patients and offer brief intervention counseling services on a limited basis prior to- or in conjunction with a referral to a formal behavioral health program.
    • Provide targeted assessment and evaluation, including diagnostic impressions and functional status focused on the presenting problem.
    • Provide timely and succinct feedback to Primary Care Manager's (PCMs) regarding consultations findings and recommendations.
    • Assist with implementation and monitoring of treatment plans with Primary Care Manager's (PCMs) and Behavorial Health Case Manager's ( BHCMs).
    • Use objective and measurable outcomes to evaluate treatment effectiveness and make adjustments as appropriate.
    • Work as a primary care team member to develop specific clinical pathways or best practice programs for targeted patient groups (e.g., patients with acute stress symptoms and PTSD).
    • Perform bio-psychosocial assessments on behavioral health patients to determine a diagnosis.

    • Must be able to obtain and maintain current Basic Life Support (BLS) certification through the American Red Cross. Advanced certification (e.g., ALS) does not supersede BLS.
    • Applicants must be able to fulfill credentialing requirements and obtain and maintain appropriate/relevant clinical privileges.
    • Immunization Screening is required.
    • Tuberculosis Test is required.
    • Require Pre-Employment Physical.
    • Lifting requirement up to 40 LBS.

    Basic Requirement for Social Worker (Clinical)

    Degree: Master's degree (or higher degree) in social work from a school accredited by the Council on Social Work Education. You must submit a copy of your transcript.

    In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:

    Specialized experience: One year of specialized experience which includes conducting bio-psychosocial evaluations, providing individual or group clinical counseling, intervention therapy or familiarity with the Internal Behavioral Health Consultant role and developing treatment plans and/or recommendations for behavioral health patients.

    Some federal jobs allow you to substitute your education for the required experience in order to qualify. For this job, you must meet the qualification requirement using experience alone-no substitution of education for experience is permitted.

    Licensure: A current, active, valid, unrestricted clinical license to practice Social Work independently, offered by a U.S. State, District of Columbia, commonwealth, territory or jurisdiction. You must submit a copy of your license.
