Visiting Nurse Association of Northern New Jersey (VNANNJ) is seeking a full-time Home Care Intake Coordinator. The selected candidate will provide assessment of patients’ home health care and hospice needs and coordination of referrals to home care. Documents all requests for services to all VNANNJ programs. This position is based in our Morristown, NJ office 50% of the time and works remotely 50% of the time.

Responsibilities for a Home Care Intake Coordinator include:

• Informs patients, families, and other agencies of the array of services available.
• Receives direct telephone and fax request orders for nursing, rehabilitation, home health aide services and agency services. Coordinates dates for first visit based on patient need and/or physician request.
• Conveys information regarding home care and hospice care services.
• Enters all patient information received via fax or phone into computer.
• Contacts referral source to clarify patient information and/or obtain additional clinical information in order to submit a complete referral.
• Reviews weekend report to follow up on any scheduled weekend admission not completed.
• Communicates information to managers and nursing personnel as needed.
• Obtains initial insurance verification for new admissions.
• Reviews all new managed and private insurance admissions for accuracy of insurance information/verification and requests initial authorization for services.
• Reviews all managed and private insurance requests for reauthorization and requests ongoing authorization.
• Verifies all patients insurance benefits monthly.

Qualifications for a Home Care Intake Coordinator:

• Bachelor's degree required
• One to three years’ experience in the home health care field preferred
• Knowledge of managed care and Medicare/Medicaid and commercial billing/reimbursement preferred. Excellent communication and presentation skills.

Pay: $27.00 - $30.00 per hour
