Job description

A local non-profit in-home care services agency is looking for an experienced Case Manager to administer admission and supervision of our 245D waivered services.

Responsibilities include:

• Completing screenings/intakes for new clients
• Developing and/or writing and implementation of each client’s person centered support plans including (CSSPA) Coordinated Service and Support Plan Addendum, (SMA) Self-Management Assessment, and (IAPP) Individual Abuse Prevention Plan.
• Staying in compliance with 245-D licensing rules and regulations.
• Performing quarterly, semi-annual and annual reviews as needed or directed by the person served and/or team in order to person-centered services.

Qualification:

Bachelor's degree in a field related to human services AND 1 year full-time experience providing direct care to individuals with disabilities or to those who are age 65 or older

• OR an Associate's degree in a field related to human services or public relations AND 2 years of full-time experience providing direct care to individuals with disabilities or to those who are age 65 or older
• OR a diploma in a field related to human services from an accredited post-secondary institution AND 3 years of full-time experience providing direct care to individuals with disabilities or to those who are age 65 or older
• OR a minimum of 50 hours education and training related to human services and disabilities AND 4 years of full-time experience providing direct care to individuals with disabilities or to those who are age 65 or older under the supervision of a person who meets any of the qualifications above.

Bi-lingual in Russian preferred.

At least 2 years of previous hands-on experience as a 245D designated coordinator managing IHS/ICLS services required.

Job Type: Full-time

Pay: $65,000.00 per year
