Hudson Valley Regional Community Health Centers (HVRCHC) seeks a Community Relations/Outreach/Patient Navigator for our health center. This is a full time, benefit eligible position. Bachelors degree or comparable education and experience required; at least two years' experience in outpatient health care,non-profit outreach,health promotion or commensurate experience; knowledge of trends and issues facing members of the community and populations served by HVRCHC. Proficiency in MS office suite and social media platforms required.

Major responsibilities include:
Developing and maintaining relationships with community based organizations, public and elected officials and referral sources in alignment with HVRCHC's services;
Coordinating with health center personnel and outside providers, when appropriate;
Triaging external referrals;
Promoting and providing information about the health center at community events;
Developing and distributing outreach materials to all interested parties and community groups;
Greeting new patients and existing patients and assisting the front desk as needed;
Coordinating intake interviews for new and existing patients and records patient information in to the EHR as assigned;
Obtaining patient documents, medication lists, and medical, family and social histories and recording and reconciling them accurately into the EHR;
Coordinating and assisting patients in identifying appropriate health and social support networks including medical insurance if appropriate;
Assisting with the interpretation and translation of non-English speak patients as appropriate;
Communicating professionally and demonstrating sensitivity to the needs of all people served at HVRCHC and maintaining patient privacy and confidentiality by abiding by HIPAA rules and cofidentiality regulations
