Community Health Director is responsible for developing plans for community-based health programs aimed at the prevention of disease and promotion of health. Recommends community health initiatives, policy implications and best practices after reviewing health literature and statistics. Being a Community Health Director identifies community health program related grant funding and develops grant proposals. Manages specific community health programs from planning and design through implementation and evaluation. Additionally, Community Health Director requires a master's degree in a related area. Typically reports to a top management. The Community Health Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Community Health Director typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function.     4. Meets regularly with the Clinic Administrator and designated staff to help identify and/or communicate community issues, feedback, service needs, events, and the Community Relations Department’s plan to engage providers and staff in support of Healzone strategies and outcomes.
    5. Comply with expectations of grant requirements for the purpose of statistical tracking to generate monthly reports. Use Electronic Medical Records as a tool to assess chronic illness management and improvement.
    6. Serve as a liaison between the Community Relations Department, health center staff, and the community while continuing contact with social networks and community partners.
    7. Uses prospecting methods to generate community partnerships including initiating personal/business contacts, sourcing expansion opportunities through existing and new accounts, cold calling and educational presentations.
    8. Provide and support patients with resources, education, and referrals for the purpose of assisting patients with their chronic disease management.
    9. Attend key community meetings and events as a representative of assigned clinic, for the purpose of networking to build contacts, incorporation of HealZone strategies and community change outcomes.
