Job Overview:
Group Facilitator who delivers the Core Elements of the WILLOW intervention. Provides Health Education within the community through outreach as well as inreach efforts. Networks with community based organizations and attends health fairs.
Qualifications:
Required:
- A minimum of 2 years of experience working in HIV/AIDS prevention, healthcare systems, outreach and health promotion
- Must be trained to deliver WILLOW intervention. Must also be trained in the NJ State Department of Health and TB services required trainings as a provider to People living with HIV/AIDS. The ability to speak and lead group discussions is a plus.
- Equipped to work with multicultural and diverse populations
- Demonstrated ability to work independently with accountability; exercise sound judgment, discretion, and professionalism at all times
- Demonstrated ability to effectively collaborate with community stakeholders
- Excellent written and verbal communication skills
Preferred:
- Bachelor s Degree in a related field preferred
- Active NJ Certified HIV Counselor preferred
- PrEP Training preferred

Essential Functions:
- Conducting client-centered PrEP/PEP education and adherence counseling services with a focus on client acquisition, engagement and retention.
- Providing patients and clients with basic introductory education regarding PrEP/PEP.
- Screening / Recruiting high-risk patients and clients into PrEP/PEP education.
- Providing assistance to clients with finding a medical provider and navigating through medication payment options including patient assistance programs, insurance enrollment and prior authorizations.
- Developing and maintaining individualized treatment adherence plans.
- Conducting routine follow-up to offer adherence counseling.
- Providing outreach to all relevant communities.
- Designing, implementing, and distributing advertising and educational campaigns for PrEP/PEP.
- Ensure the ongoing collection, collation and organization of data related to resource and referral information for providers prescribing PrEP/PEP.
- Promote and participate in related community and health events as needed.
- Provide HIV Testing as needed.
- Recruit for and facilitate Kiss & Tell educational workshops for designated priority populations.
- Develop close working relationships with leadership and direct staff at healthcare providers to ensure coordination of services.
- Maintain accurate and complete participant records, tracking contact, participation, outreach, counseling, testing, and referrals performance.
