Organizes community members to work toward a common goal supported by staff and volunteer workers. Additionally, Community Organizer may require a bachelor's degree. Typically reports to a supervisor or manager. The Community Organizer work is closely managed. Works on projects/matters of limited complexity in a support role.     Job Details

     

    The Community Health Worker is responsible for identifying high risk prenatal and postpartum women and assist these individuals in gaining access to primary and preventive health services including prenatal care, PCAP, WIC, and other essential and supportive services as needed.

    Requirements:

    • High School Diploma or equivalent.
    • Valid New York State Driver's license, personal car to carry out employment duties and an active automobile insurance policy. 
    • Basic computer skills. 
    • Good writing and verbal skills. 
    • Spanish or Creole speaking a plus. 


    Salary Range

    USD $22.05 - USD $22.05 /Hr.



    This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
