CASE MANAGER JOB DESCRIPTION
Nature of Work:
The Case Manager is responsible for assisting program participants achieve success in completing program requirements. Duties involve meeting with participants on a regular basis, and providing reports to the Court concerning participant compliance.
Examples of Work:

Conduct participant intake services into the Recovery Court Treatment Program, providing case plan referrals for housing, employment, skill training, medical, etc.
Serves as liaison with the program’s primary care and ancillary service providers (i.e. housing, employment, educational opportunities, and medical, legal and social services)
Monitors and evaluates participants’ behavior to ensure program compliance, including drug screen participation/results, educational requirements, frequent verification of employment, and public safety.
Maintains frequent communications with treatment staff to monitor compliance.
Provides case management reports at weekly participant staffing and status hearings, to include reports from all facilities participating in participants’ recovery or educational endeavors.
Maintains required statistical participant information for monthly reporting to the Louisiana Supreme Court.
Evaluating participants’ progress periodically and making adjustments as needed to improve outcomes.
Following up with discharged clients to ensure they were satisfied with services and they are still in good physical and mental health.
Performs all other duties as assigned.
