Job Description:
Provide face-to-face, telehealth and phone crisis services. Assist Crisis and Access Services and Medical Services in engaging clients in treatment services, screening and following up with high risk clients, assisting with care coordination and life skills as needed during transition to other services, and connecting clients to necessary resources.
Supervised By: Vice President of Clinic Outpatient and Engagement Services
Job Qualifications: At least two years of direct service experience working or volunteering in a human services organization. Candidate will be flexible, organized, detail-oriented, and experienced in behavior programs and able to work well within a team setting. Bachelor's degree required.

Supervisory Responsibilities for: This title does not supervise others.
Complete clinical documentation within one business days of the service provided.
Complete treatment plans and other clinical service authorizations in a timely manner.
Regular and Reliable Attendance is required.

All other duties as assigned.
