Targeted Case Manager
Mountain Comprehensive Care Center is hiring a Targeted Case Manager to serve Garrard County.
Case Managers provide additional support to clients active in our treatment programs by connecting them with additional resources or attention to improve their quality of life. The role requires excellent verbal and written communication skills, self-sufficiency with independent judgment for prioritizing and completing tasks, and the ability to succeed in a teamwork environment to deliver quality care.
Responsibilities:
· Document client history with specific skill deficits, basic support needs, levels of functioning, risk status, define the scope and focus of the service plan
· Assess and prioritize the needs and strengths of the population served
· Collect information from medical professions, educators, social workers and other sources including but not limited to family members and friends
· Create care plans that identify the mission and vision of the client as well as identifying goals and objectives
· Document notes with details of description of services provided and involvement & performance towards care plan goals
· Coordination resources and refer with organizations in the community
· Document direct and indirect contacts with the patient
· Communicate effectively pertinent information in cases with clinical staff
· Monitor care plans, progress notes and review of activities and contacts to obtain goals with clinical services supervisor
· Attend care plan meetings
· Maintain a caseload of 25
Qualifications
· Minimum: Bachelor’s degree in human service field, (i.e. Psychology, Social Work, Counseling) with a minimum of one year experience. Minimum of one year Case Management experience required or a Master’s in human service may substitute.
