Diagnosis Related Group (DRG) Coordinator jobs in Erie, PA

POSITION SUMMARY:

Position responsible for the treatment and care coordination for clients at Gage House. Duties include: level of care assessments, intake/orientation, developing and implementing preliminary and comprehensive treatment plans, as well as providing individual and group counseling to provide clients with the opportunity to work towards treatment goals and explore recovery. The incumbent will also coordinate aftercare services for services after discharge. The incumbent is responsible for the documentation of these services within the timelines designated by the agency and regulatory bodies.

MINIMUM QUALIFICATIONS:

1. Bachelors level degree in related field or equivalent education and experience as determined by agency standards and guidelines of position.
2. Exceptional communication skills, both written and verbal, organizational skills and the ability to effectively work with a team.
3. Strong clinical skills with working knowledge of mental health system, drug and alcohol system and recovery principles.
4. Ability to effectively work with persons with mental illness and develop comprehensive treatment / recovery plans for their program participation.
5. Act 33/34/31 and FBI Clearances.
6. Valid driver's license with access to a vehicle during scheduled work hours.
