POSITION SUMMARY:
Position responsible for the treatment and care coordination for clients at Gage House. Duties include: level of care assessments, intake/orientation, developing and implementing preliminary and comprehensive treatment plans, as well as providing individual and group counseling to provide clients with the opportunity to work towards treatment goals and explore recovery. The incumbent will also coordinate aftercare services for services after discharge. The incumbent is responsible for the documentation of these services within the timelines designated by the agency and regulatory bodies.
