Under supervision by the PACE Program Manager, plans and provides nutritional care for Siouxland PACE participants as a member of the Interdisciplinary Team. Assesses nutritional needs, develops and implements nutritional care plans, and evaluates and reports these results appropriately. Responsibilities:
- Maintains a working knowledge of applicable PACE Federal, State, and local laws and regulations, the Compliance Accountability Program, Code of Ethics, as well as other policies and procedures in order to assure adherence in a manner that reflects honest, ethical, and professional behavior.
- Obtains and evaluates nutritional and medical history, laboratory data, test results, current medications, current diet orders and nutritional regimen to determine participant’s estimated nutritional needs.
- Recommends and implements nutrition interventions that assist in maintaining or improving nutritional-related medical diagnoses.
- Completes nutrition assessments for participants per regulatory guidelines and when needed to assist in developing participant’s plan of care and service delivery requests.
- Instructs participants, family members, staff and contracted providers/facilities on diet, food safety and sanitation.
- Documents pertinent participant information in the electronic medical record (EMR) and the participant plan of care.
- Attends Interdisciplinary Team meetings daily and team activities when scheduled.
- Coordinates meal planning/delivery for PACE Center meals with contracted food vendor monthly.
- Monitors food safety and sanitation of PACE meal service.
- Performs other duties as assigned.

- Employee maintains current licenses and/or certifications required for the position.
- Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.
- Completes all annual education and competency requirements within the calendar year.
- Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.
Qualifications:
Education
Bachelor of Science degree in Dietetics and/or food and nutrition that meets or exceeds the requirement by the American Dietetic Association for the registration examination.
Successful completion of internship or coordinated undergraduate program that meets the American Dietetic Association guidelines.
License(s)/Certification(s)
- Registration or registration eligible with the Commission on Dietetic Registration
- Licensed or licensed eligible with the State of Iowa
- Current CPR/BCLS certification
- Maintains current Adult Mandatory Reporter status
- Maintains American National Standards Institute accredited food protection/safety certification (i.e. ServSafe Food Protection Manager/Certified Food Safety Manager) or obtained within 90 days for new hire.
- Valid driver’s license
- Valid Auto insurance
Knowledge/Skills/Abilities
Effective oral and written communication skills to interact effectively with PACE participants, their families, and other health care professionals
Ability to effectively communicate and work within the Interdisciplinary Team setting.
Strong organizational skills.
Able to manage changing priorities per needs of the program
Use of Microsoft office products.
