New Franklin Center for Rehabiliation and Nursing is seeking a Dietitian - flexible schedule.
Join our talented team of Dietitians in providing excellent nutrition and dietary services for our long term residents and short term patients.
DUTIES:
• Assess the Residents' nutritional status, provide recommendations to medical staff for diet, and develop nutritional care plans
• Evaluate special needs of Residents regarding nutrition support, skin breakdown, mood and significant weight issues
• Monitor Resident meal service to ensure diet modifications are followed
• Educate Residents, families, and staff in concepts of nutrition & diet modification
• Routinely evaluate Dietary Care plans for effectiveness to ensure high quality of care
• Participate with an interdisciplinary team to review and adjust residents' plan of care
• Work closely with Speech-Language Pathologist to ensure appropriate mechanically altered diets are provided for residents with dysphagia
• Reviewing all menu changes to ensure they follow the facility's policies/procedures & State/Federal guidelines
• Recommend diet order and menu changes in computer menu software
• Complete required assessment and documentation such as assigned MDS areas and periodic assessments
• Monitor residents weight and food/caloric intake
• Communicate with an interdisciplinary team on a daily basis to provide quality care to Residents
- Complete assigned sections of MDS and other assessments as needed
REQUIREMENTS:
Registered Dietitian or eligible
Nursing Home, Long Term Care or Acute Care Experience preferred, new grads considered
Knowledge of applicable state and federal guidelines
Computer literacy and proficiency with EMR (Sigmacare) software and computer-based menu systems (Gerimenu) helpfull - not required
