  • Job Summary

    The Director of Community Health Services is at the Oyate Health Center. This position is to assess, plan, identify outcomes, direct, and administer a comprehensive community health program for the communities served by the Oyate Health Center.

    Essential Functions

    • Performs community assessments and tracks community health status. Evaluates health trends and risk factors of populations groups and directs the programs to improve population health.
    • Investigates health problems and hazards such as communicable diseases and develops a recommended plan of action to address these issues.
    • Implements and directs an overall community health nursing program and plans to satisfy varying patient load requirements, priorities for service and other healthcare objectives.
    • Collects data in a systematic and organized manner, i.e., demographic, immunization, and analyzes the collected assessment data. Prepares regularly scheduled progress reports and special reports as required.
    • Plans work to be accomplished by subordinates and adjust short-term priorities and evaluates work performance. Gives advice, counsel, or instructions to employees on both work and administrative matters.
    • Assesses the health needs of individuals, families, and the community serves taking in to account the total situation in the family, the home, and in the community, environment including such considerations as educational level, income level, sanitary practices and hygiene conditions, psychological adjustment, and resources available.
    • Interviews and observes the patient, family members, and others, observes the environment, studies the medical diagnosis and treatment plan when one has been made; and interprets tests previously administered. Evaluates these factors as they relate to each other, to the patient, to the family, and to the community.
    • Recognizes symptoms that indicate the early stages of serious physical, emotional, or mental problems and other needs for medical care of all family members, for recording the details of such cases, and for referring them for medical follow up.
    • Provides health education to individuals, families, and community groups in various settings.
    • Performs community assessments to develop a plan of action.
    • Collaborates and establishes partnerships with Tribal Community Health Partners, Community Partners, Grant Programs, Local, State, and Federal systems.

    Supervisory Controls

    The supervisor provides administrative direction with assignments in terms of broadly defined goals or functions. The employee usually has responsibility for independently planning, designing, and carrying out programs, projects, studies, or other work. Results of the work are considered technically authoritative and are normally accepted without significant change. If the work should be reviewed, the review concerns such matters as fulfillment of program objectives, effect of advice and influence on the overall program, or the contribution to the advancement of technology. Recommendations for new projects and alteration of objectives usually are evaluated for such considerations as availability of funds and other resources, broad program goals, or established priorities.

    Guidelines

    Guidelines for performing the work are scarce or of limited use. Administrative policies, precedents, laws and regulations are applicable but are stated in general terms. The employee uses initiative and resourcefulness in deviating from traditional methods or researching trends and patterns to develop new methods, criteria, or proposed new policies.

    Complexity/Scope of Work

    The work typically includes varied duties that require many different and unrelated processes and methods, such as those relating to well established aspects of an administrative or professional field. Decisions regarding what needs to be done include the assessment of unusual circumstances, variations in approach, and incomplete or conflicting data. The work requires making many decisions concerning such things as interpretation of considerable data, planning of the work, or refinement of the methods and techniques to be used.

    The work involves establishing criteria; formulating projects; assessing program effectiveness; or investigating or analyzing a variety of unusual conditions, problems, or questions. The work product or service affects a wide range of organizational activities, major financial activities, or the operation of other programs divisions of the organization.

    Contacts

    The personal contacts are with individuals or groups from outside the organization in a moderately unstructured setting. For example, the contacts are not established on a routine basis and the purpose and extent of each contact is different.

    Contacts are primarily for obtaining, furnishing, exchanging and providing factual medical information.

    Physical Demands / Work Environment

    The work requires some physical exertion, such as long periods of standing; walking over rough, uneven, or rocky surfaces; recurring bending, crouching, stooping, stretching, reaching, or similar activities; or recurring lifting of moderately heavy items. The work may require specific, but common, physical characteristics and abilities, such as above average agility and dexterity. The employee must be able to read, write, speak and hear.

    The work involves moderate risks or discomforts that require special safety precautions, e.g., working around moving parts, carts, or machines; exposure to contagious diseases or irritant chemicals. Employees may be required to use protective clothing or gear, such as masks, gowns, coats, boots, goggles, gloves, or shields.

    Supervisory and Management Responsibility

    This position is a first level supervisor and is usually responsible for the work performance of a small group of employees or a larger group with a homogeneous objective (i.e., employees all perform same basic type of work). The employee is responsible for planning, organizing and monitoring day-to-day work on a -short-term- cycle. This position assigns work to subordinates, adjusts workflow to maintain balance among positions and meet priorities or deadlines, and makes minor changes in structure, methods, or procedures as necessary to accommodate changes in work pattern, emphasis, or capability. The employee may recommend major changes for higher level action. The employee usually counsels with employees and hears and resolves minor complaints and grievances and participates in performance evaluation and personnel management recommendations.

    Minimum Qualifications

    • Master’s degree and three (3) years of relevant experience, or bachelor’s degree and five (5) years of progressively responsible relevant experience.
    • The experience required for supervisory/management positions will be inclusive of at least three (3) years of supervisory/management experience.
    • This is a senior professional and/or management position that requires post-secondary education and/or considerable experience or a clinical degree requiring a clinical rotation/internship.
    • Individuals must have applicable education and/or experience applying principles, concepts, and methodology of a professional or administrative occupation to permit the independent performance of recurring assignments or carrying out difficult/complex assignments, operations, and procedures; or applicable education and/or experience applying a wide range of technical methods, principles, and practices similar to a narrow area of a professional field such as the design and planning of difficult, but well-precedented projects.
    • Must successfully pass a pre-employment drug screen, criminal, and background check.
