    The Discharge Planner is a vital part of Arbour Hospital’s Partial Hospital Program team. This is a full time, Monday – Friday, 40 hour per week position at Arbour's main campus in Jamaica Plain, MA.

    Responsibilities
    • Evaluate and understand the unique needs of each patient
    • Identify resources and treatment options accepted by patients’ insurances
    • Complete timely discharge plans under supervision of the program manager
    • Document case management and discharge planning appropriately; accurately reflecting the progress of the client and treatment planning goals
    • Collaboratewith patients’ community care teams and existing providers
    • Attend treatment team meetings when indicated
    • Complete follow-up calls to scheduled and current clients

    Qualifications:
    Requirements
    • Bachelor's in human services field (psychology, criminal justice, sociology, social work etc); BSW is preferred
    • Knowledge of community resources and familiarity with mental illness and substance use is required
