Rolling Hills Hospital is hiring a Discharge Planner to join the Clinical Servives team. The Discharge Planner facilitates inpatient discharges to the next level of care with a focus on transitioning care from inpatient setting to RHH outpatient programs. The Discharge Planner provides groups and individual education to patients on the importance of continuing care. This position is based in the outpatient services building but will spend a majority of time with inpatient populations. 

Experience:

·        Experience in health care business development/marketing environment with measurable business development results; experience in acute inpatient and outpatient psychiatric setting; Strong assessment skills required; excellent telephone etiquette and tact; ability to interact effectively with persons of widely diverse roles, backgrounds, cultures, and socio-economic classes; effective oral and written communication skills; basic data entry skills preferred; organizational, time management, problem solving, crisis management skills necessary; flexibility, creativity, and ability to manage stress necessary.

Education:

·        Master’s degree in a Behavioral Health related field (e.g. Counseling, Social Work, Clinical Psychology) required
