University Behavioral Center, a subsidiary of Universal Health Services, Inc. Our mental health facility provides safe and effective inpatient treatment to assist individuals in leading more productive and fulfilling lives. The facility is located on a beautiful 14-acre campus in Orlando, Florida, and includes 112 licensed beds. We are currently seeking qualified professionals with active/clear license in the state of Florida. Previous experience in a psychiatric facility or rehabilitation program is required.
POSITION SUMMARY:
The Discharge Planner is responsible for case management and coordination of discharge planning services for patients within the inpatient units. Rounding daily with patients to ensure that discharge plans are relevant and aftercare appointments are established prior to discharge date. Establish relationships with community providers and coordinate aftercare plans specific to patients’ needs. The Discharge Planner is also liaison to communicate with the MD and the court system in order to establish any extended commitments and to ensure that court proceedings occur on the premises. This role includes communication with the physicians to obtain second opinions and appropriate signatures for documents within the allotted time frames. The Discharge Planner will facilitate the development of discharge plans within the Master Treatment Plan. This is a Per-Diem position.
