Qualifications

• Education: Bachelor’s Degree in Social Work, current WI Social Work licensure required
• Experience: Prefer 2 years social work experience in case management or health care setting
• Opportunity for clinical supervision working toward LCSW licensure.

Key Responsibilities: Job Specific Duties

• Attends daily Quick Check report.
• Participates as a team member in the Interdisciplinary Treatment Planning conferences.
• Reviews admission packets/consents/visitation rights and information with patients and families upon admission.
• Facilitates meetings and education lectures for patients/family members as requested.
• Provides direct social work services to patients and their families in relation to seeking alternative placements following discharge such as locating financial assistance and community services, and coping with anxieties and stress precipitated by illness and hospitalization in conjunction with the Clinical Therapist at Inpatient BH.
• Coordinates family/patient conferences weekly.
• Maintains constant communication within the department, between other departments, and with Inpatient Treatment Team
• Participates in public relations, community education and develops referral sources.
• Provides cross-coverage for BH Intake Coordinator and/or Amery Hospital Social Worker as needed.
• Participates in Utilization Review/monitors length of stay and addresses concerns with Providers and Program Director as needed
