Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager.     Responsibilities:
    • Visit patient homes and update patient care binders.
    • Ensure patients have the proper durable medical equipment in their homes. 
    • Collect and perform supervisory visit surveys. 
    • Effectively communicate and provide information to patients, caregivers, and physicians. 
    • Understand HIPAA and maintain confidentiality.
    • Display a warm and friendly attitude.


    Minimum Qualifications:
    • Strong work ethic and writing skills.
    • Dependable transportation. 
    • Experience with MS Office products (Outlook, Word, Excel, etc.)
    • Minimum associate level education preferred. (High school diploma or equivalent is required.) 
    • 1 year of medical knowledge preferred.
