Being a Disease Management Case Manager acts as a liaison between the participant, employer, and physician. Stays abreast of healthcare changes and documents the patient's progress. Additionally, Disease Management Case Manager typically requires a bachelor's degree in nursing. Requires a Registered Nurse (RN) license. Typically reports to a manager. Case Management Nurse Manager will take on the role of serving as a patient advocate, and act as a liaison between all patient care services to ensure optimal patient experience.
It is highly preferred, but not required, that the RN Manager Case Management candidate has 1 year of nurse leadership, and case management experience.
