A Magnet designated acute care hospital located in central Connecticut is currently searching for an experienced Registered Nurse RN Case Manager to join their award-winning healthcare team.

Reporting to the Director of Case Management, the RN Case Manager will provide clinically-based care management, utilization review, and discharge planning services. The RN Case Manager will partner experienced Social Workers and other healthcare professionals to identify appropriate utilization of resources, ensure timely and accurate reimbursement, create comprehensive discharge plans in accordance to patient needs, and educate both patients and families.

Requirements Include:

• Bachelor’s degree in Nursing BSN
• Current Registered Nursing licensure in state of Connecticut
• Prior experience in acute care case management or utilization review
• Nursing experience in a Medical-Surgical setting preferred
