Summary:
Under the guidance and supervision of the Director of Care Management Services Division or designee, the Coordinator / Outreach Engagement provides supervision and guidance to the program staff and outreach/engagement services to program participants who need assistance with medical, behavioral health and social services in the community.

Health Homes of Upstate NY and St Peter’s Health Partners
- Identifies individuals in the community in need of medical, behavioral health, and social services.
- Contacts identified individuals and assigned individuals referrals within 3 days of receiving the assignment and within 24 hours when the individual referred is in the hospital or homeless settings.
- Verifies that the referred individual meets program requirements including having an active Medicaid case.
Competes all required paperwork with the referred individuals and assigns them to the program Care Managers for health home services.
- Maintains regular contacts with participants in the program including home visits as needed.
- Maintains accurate and updated records of each participant of all services provided.
- Ensures compliance with program policies and procedures as required by funders.
- Conducts monthly audits of program as assigned by supervisor.
- Prepares and distributes detailed reports of audits conducted.
- Prepares and submits monthly report of all audit activity conducted during the month.
- Provides supervision and support to program staff and prepares Performance Appraisals.
- Attends staff meetings and formal/informal training sessions in/outside the agency.
- Participates in community activities as representative of the program when assigned.
- Comfortable traveling between four counties (Montgomery, Schenectady, Rensselaer, and Albany)
- Is available to work outside regularly scheduled work hours as needed.
- Any other duties as may be assigned by the immediate supervisor.
Authority:
The employee exercises initiative and some independence in establishing his/her schedule for meetings with participants, other care managers, and community agencies. Follows pre-established procedures for program implementation and daily operations. The individuals must be resourceful and able to assess situations to determine if a crisis is involved and how to best proceed within the framework of the agency’s as well as the program’s policies and procedures. The employee will consult with supervisor if in doubt prior to proceeding.
