    The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center.
    PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.

    Primary Purpose
    Responsible for all activities related to program promotion and participant intake and enrollment. Evaluates potential enrollees to determine participant's needs and to determine if the individual meets eligibility criteria in accordance with current existing federal, state, and local standards, as well as established policies and procedures. Lastly, this role will annually reassess current participants for continued eligibility and enrollment.

    • Interdisciplinary Team (IDT) Representative
    Duties and Responsibilities
    • Provides information to potential enrollees, families, caregivers, referral sources, etc. regarding Senior Care Partners P.A.C.E. and the services provided.
    • Conducts in-home intake assessments with potential enrollees and families to determine participant needs and potential eligibility.
    • Reviews Medicaid eligibility status for referral for Medicaid benefit consultation as indicated.
    • Completes face-to-face Michigan Medicaid Level of Care Determination assessments at time of initial assessment, annual redetermination, and significant change of condition.
    • Monitors assessment process to assure timeliness in processing new applicants.
    • Follows up with and tracks referral source activity within 72 hours of intake assessments.
    • Completes appropriate documentation related to participant assessments in EHR as required.
    • Works in conjunction with the IDT team to inform about potential enrollees needs and eligibility.
    • Reviews Participant Handbook, including benefits, grievances, and Bill of Rights.
    • Enters Level of Care Determination in CHAMPS as required.
    • Enters enrollments in CHAMPS as required.
    • Maintains statistical data as required including intake log and status reports.
    • Develops and maintains a referral network to include area service providers, senior service organizations, local community agencies and leaders.
    • Regularly evaluates referrals and makes continuous improvement based on feedback.
    • Introduces the P.A.C.E. concept by holding education community presentations as needed.
    • Arranges and coordinates presentations to a wide variety of audiences as required. Includes key staff members in community education activities.
    • Works effectively with Intake and Enrollment team as necessary.
    • Participates in all appropriate program teams and committees.
    Qualifications
    • Must be a licensed/certified physician, registered nurse, licensed practical nurse, social worker, physician’s assistant, physical therapist, respiratory therapist, occupational therapist, or speech therapist.
    • Minimum of one year experience working with the frail, elderly, or long-term care population.
    • Must have experience in a health care setting working directly with individuals.
    • Minimum of 2 years of experience working with sales-related field that benefits elderly in a community-based setting required. Must have knowledge of senior community and area and area resources.
    • Must have strong working knowledge of state and federal programs that provide medical care and financial support to individuals.
    • Works independently with a strong, organized sense of focus.
    • Excellent oral and written skills required. Must be able to demonstrate computer literacy.
    Work Environment
    • Has a visible presence while working in the center and is subject to frequent interruptions.
    • Communicates with staff, participants, caregivers, the board of Senior Care Partners P.A.C.E., and other organizations as necessary.
    • Works beyond normal working hours when necessary.
